The following information
must be given/ reported :
- Your name
- Your insurance policy number
- The name of the insured person who called
- A brief description of the problem
- The information of the different actors
(policy holder, insured
person, hospital, doctor,…)
What
to do to be reimbursed?
Send the following documents to the PO Box 13, 1050 Brussels
5, Belgium.
- The statement of damage which is available on
our website
- The original invoices and detailed costs in case
of full cover
- A copy of the invoices and detailed costs with
the certificate of the National Health
Service in case of TOP UP
- A medical certificate that mentions the diagnosis
What
to do in case of modification of the administrative data?
Send following

It's necessary
to inform the company in the following cases:
- Change in family situation
- Change of address
- Change of statute of social security (modification
form E)
- Change of country of residence
- Change of nationality
- Change of the covered region
- Change of cover
- Change of deductible
- Change of all other administrative information
In every conversation
in all cases, we need the following information:
- Your name
- Your policy number
- Name of the insured person
